Use this form to request ADA Accommodation. We will respond to ADA accommodation requests within 15 days. Please request event accommodations at least 7 days in advance.

The ADA Circulation Qualification Program serves patrons who have disabilities that are expected to last at least six months. Applicants must have a condition that makes it difficult or impossible to return Library materials within the usual checkout periods: Two weeks for DVDs and Peak Picks and three weeks for other materials.

ADA Circulation Qualification status

**A maximum of 30 items may be checked out and placed on hold as a qualified ADA Borrower. Patrons seeking ADA Circulation Qualification Status accommodations above the 15-item limit to a maximum of 30 items, may be required to provide healthcare certification regarding their self-disclosed disability.

The ADA Circulation Program offers the following accommodations:

  • 4-week loan for all regular circulating items (including DVDs and Peak Picks)
  • 4-week renewals for all renewable items (Peak Picks are not renewable)
  • Hold expiration period of 14 days
  • Maximum check out limit of either 15 items or 30 items
  • Maximum hold limit of either 15 items or 30 items
  • Exception from account suspension when item is 14 days overdue (account will be suspended if an item is lost or 31 days overdue)
  • Exception from The Library's collections/material recovery process

The Americans with Disabilities Act (“ADA”) does not require The Seattle Public Library to take any action that would fundamentally alter the nature of its programs or services, or impose an undue financial or administrative burden.

We will respond to your request as soon as we can, but no later than 15 days after we receive it. Please note that certification by a health care provider may be required. If you are not satisfied with the response to your request, you may appeal to the Director of Administrative Services at 1000 Fourth Avenue, Seattle, WA 98104, within 15 days of receiving the response.
